Who Is a Shared Family Living/Enhanced Family Care Provider (Provider)?
A Shared Family Living/Enhanced Family Care Provider (a Provider) is/are individual(s) (eighteen years of age or older) contracted to support the lifestyle, needs and safety of an individual with disability in family style environment. Examples of Providers are:
Parents
Relatives
Family friends
Community members with caring hearts
As a Provider, you will:
Complete screening, training, background checks, home certification, paperwork and all requirements by the state of New Hampshire to become a vital part of an individual’s community
Enrich the life of someone who deserves love, dignity, and opportunity to live their highest quality of life
